What is Root Cause Analysis (RCA)?

Any time an unwanted event keeps happening over and over again, you need to fix it once and for all. It is very much like an unwanted plant that keeps growing, even after you feel like you pulled it out by its roots. So you need to get down to the nitty-gritty of the cause of the problem. 

In Six Sigma, we use Root Cause Analysis to dive deep into solving problems. Some of the additional tools that help include:

• 5 Whys
• Fishbone Diagram
• Pareto Chart

With these tools and the template shown, you will be able to get to the root of any problem.
